We are in network with all major insurance plans with the exception of OHP/Care Oregon. We submit insurance for Motor Vehicle Accident claims.

If you wish for Centered Healing to bill insurance on your behalf, we require that you complete the Insurance Eligibility and Verification form prior to your first appointment or at least within one week of your first appointment.

Please call your insurance plan and ask the questions listed on the form and record the information that they are able to provide. This information helps you understand your coverage for Acupuncture services, what your responsibility will be, enables us to bill according to your coverage and reduces the chance of claim denials. 

Note: Some plans require a pre-authorization for Acupuncture benefits. It is the patient’s responsibility to know the details of their plan coverage, to inform us if they need a pre-authorization and we will complete the necessary paperwork to acquire the authorization. (This takes 2-3 business days).

If you have been in a Motor Vehicle Accident, we want to treat you as soon as possible. No referral or insurance paperwork is needed. Claim information will be gathered at your first visit.
